By completing this purchase and participating in a DIY workshop at C & A Designs MN, I acknowledge and agree to the following:

Assumption of Risk

I understand that participation in DIY workshops may involve the use of tools, equipment, and materials including but not limited to epoxy resin, chemicals, heat tools, sharp objects, hand tools, and power tools. I acknowledge that these activities involve inherent risks, including but not limited to cuts, burns, allergic reactions, respiratory irritation, eye injuries, and other personal injuries or property damage.

I voluntarily choose to participate and assume all risks, both known and unknown, for myself and for any minor(s) I am registering.

Adult Participation

I certify that I am physically able to participate, have disclosed any relevant medical conditions or allergies (including chemical sensitivities), and agree to follow all safety instructions provided by instructors and staff.

Minor Participant Consent

If I am registering one or more minors, I certify that I am the legal parent or guardian of the minor(s). I give permission for the minor(s) to participate and acknowledge that the activities involve inherent risks.

On behalf of myself and the minor(s), I hereby release, waive, and discharge C & A Designs MN, its owners, employees, instructors, volunteers, and affiliates from any and all claims arising from participation, including those caused by negligence, to the fullest extent permitted by Minnesota law.

Health & Safety Acknowledgment

 

I certify that all participants are physically able to participate and that I have disclosed any relevant medical conditions, allergies, pregnancy, or limitations prior to participation. I understand that failure to follow safety instructions may result in removal from the workshop without refund.

Photo & Media Release

 

I grant permission for C & A Designs MN to use photographs or videos taken during the workshop for promotional purposes, including social media and marketing materials, without compensation, unless I notify staff in writing prior to the event.

No Refunds

 

I understand that no refunds will be issued due to injury, discomfort, allergic reaction, illness, or early departure.

Governing Law

 

This agreement shall be governed by and interpreted under the laws of the State of Minnesota.

Acknowledgment

 

By checking the box at checkout, I acknowledge that I have read, understand, and agree to this waiver for myself and for any minor(s) I am registering.
